If you are 16-26 years old and you live with a disability, the Independent Living Fund (ILF) can help you access Person-Centred Planning support by applying for their Young Persons Planning Grant.
Y.O.U. is a founding member of the Person Centred Planning Network (Scotland). Y.O.U is one of the person centred planners who can support people through the person centred planning process. To find out more about the ILF and how to apply to the Transitions fund click here
