Y.O.U and Dr Danielle Farrel are one of the two coordinators of the Hospital Discharge Impact Network in Scotland. It is our role to facilitate four network meetings over a six-month period and support the network to think about how they can make a positive lasting change in social care, specifically around hospital discharge and how their shared experiences can be brought together to play a key part in implementing these.

As part of Y.O.U’s coordinator role we also attend four coordinator meetings within the same six-month period. It is our responsibility to share our network’s discussion and learning with the other network coordinators and to identify ways in which the networks can coordinate
and work together to bring lasting change into the process of hospital discharge in the four nations of the UK.

Y.O.U looks forward to continuing in this role and also to exploring other opportunities to work in partnership with IMPACT in the future.
